Welcome to PureCare Pharmacy. These Terms and Conditions outline the rules and regulations for the use of our pharmacy services. By accessing and using our services, you accept and agree to be bound by these terms. If you disagree with any part of these terms, please do not use our services.
General Terms
- These Terms and Conditions govern your use of PureCare Pharmacy services, located at 455 School St #48, Tomball, TX 77375.
- By using our services, you agree to comply with all applicable federal and Texas state laws and regulations.
- We reserve the right to refuse service to anyone who violates these terms or engages in fraudulent activity.
- Prescription services are available only to patients with valid prescriptions from licensed healthcare providers.
Prescription Services
- Valid prescription required: All prescription medications require a valid prescription from a licensed healthcare provider.
- Prescription verification: We verify all prescriptions with prescribers as required by Texas law.
- Controlled substances: Dispensing of controlled substances follows DEA and Texas DPS regulations.
- Prescription transfers: We accept prescription transfers from other pharmacies in accordance with state and federal laws.
- Refills: Refills are processed based on prescriber authorization and regulatory requirements.
Payment and Insurance
- We accept most major insurance plans, Medicare Part D, and Medicaid.
- Co-payments and deductibles are due at the time of service.
- For non-covered medications, payment is required before dispensing.
- Insurance claim disputes should be directed to your insurance provider.
- We offer competitive cash prices for patients without insurance coverage.
Product Returns and Refunds
- Prescription medications cannot be returned once dispensed, per federal law.
- Over-the-counter items may be returned within 3 days with receipt.
- Compounded medications are non-returnable as they are customized for individual patients.
- Defective or incorrectly filled prescriptions will be replaced at no charge.
- Report any medication errors immediately to our pharmacy staff.
Service Availability
- Regular pharmacy hours: Monday-Friday 9:00 AM - 5:00 PM
- Closed on weekends and major holidays
- Emergency prescription needs should be directed to nearest 24-hour pharmacy
- Prescription processing times may vary based on insurance verification and medication availability
- Special order medications may require additional processing time
Patient Responsibilities
As a patient of PureCare Pharmacy, you are responsible for:
- Providing accurate and complete health information
- Informing us of all medications, supplements, and allergies
- Following medication instructions and asking questions when unclear
- Keeping prescriptions secure and out of reach of unauthorized persons
- Properly disposing of unused or expired medications
Limitation of Liability
PureCare Pharmacy provides services with the highest standard of care but is not liable for adverse reactions when medications are used as directed, delays caused by external parties, or non-disclosure of health information.
Prohibited Activities
We strictly prohibit fraudulent prescriptions, doctor shopping, false identification, abusive behavior, or attempting to obtain medications for illegal purposes. Violation results in refusal of service.
